The short version

Overview: Sarachnis at a Glance
Sarachnis is a level 318 spider boss in the Forthos Dungeon, entered near the Forthos Ruin in Hosidius. She has 400 Hitpoints, melees you when you stand next to her, and switches to ranged when she can't reach you. Both styles hit up to 31, and every successful hit heals her: 5 HP per melee hit and 10 per ranged hit.
Every four attacks she screeches and webs you in place for 6 ticks, then moves somewhere else in the room. At 66% and 33% health she summons two spawns. Between webs you stand next to her on Protect from Melee; during a web you switch to Missiles. Getting that switch right is most of the fight.
She is weak to crush (10 crush defence) and very strong against ranged (300), so this is a melee boss for almost everyone.

What to Pray Against Sarachnis
You only ever need two protection prayers here. Which one depends on where you are standing.

Wrong prayers cost double here: you take the hit and she heals from it. Her melee reach is also bigger than it looks, so switch to Melee before you step back in, not after.
Sarachnis Strategy, Step by Step

Pot up before the last web.
She is aggressive, so the fight starts as soon as you walk in.

Stand next to her with Protect from Melee and hit her with crush.

When she screeches "Hsss!" the web is coming.
Switch to Protect from Missiles while you are bound for those 6 ticks, and do your eating then, since you can't attack anyway.

When the web drops, switch back to Protect from Melee and walk back into melee range.

At 66% and 33% she summons a level 68 blue magic spawn (max hit 11) on the east side and a level 107 orange melee spawn (max hit 13) on the west side, each with 30 HP. Kill the blue one; tank or ignore the orange one.

She respawns 17 ticks (about 10 seconds) later.
Location and How to Get There
Sarachnis is in the burial tomb at the southern end of the Forthos Dungeon. The dungeon entrance is across the river, east of the Forthos Ruin in Hosidius.
- Xeric's talisman: Xeric's Glade puts you just east of the eastern entrance.
- Kharedst's memoirs or the Book of the Dead: Lunch by the Lancalliums lands you by the River Hos, north-east of the entrances.
- A player-grown spirit tree just south of the dungeon entrance.
The webs on the way need a slash weapon or a knife unless you wear aranea boots; the last thick web before her tomb can be passed without one. A private instance costs 25,000 coins through the thick web, or 12,500 after the hard Combat Achievements.

Aranea boots are the single best quality-of-life item. They free you from every web, so you can keep attacking and skip the prayer switch completely.

Where Sarachnis Sits in Progression
Gemstone Crab has a max hit of 1, and Brutus (the Cow Boss) teaches dodging two telegraphed specials that hit up to 19. Sarachnis is a step up from both: two attack styles that hit up to 31, a prayer switch every web, and she heals whenever she damages you.

Slayer masters who assign Sarachnis: Turael, Spria, Konar, Nieve, Duradel. Each kill is 430 Slayer XP.
For irons she is a reliable clue source and one of the few non-Wilderness places to stock red spiders' eggs through the giant egg sac. The cudgel is also a decent crush weapon for an account without a bludgeon.

- Staying on Protect from Melee while webbed, then eating her ranged hits and healing her.
- Stepping back into melee range before switching to Protect from Melee.
- Bringing a slash or ranged weapon instead of crush.
- Forgetting a slash weapon or knife for the webs on the way in.

- Eating while you could be attacking instead of saving food for the web.
